Person-Centered Planning (PCP) is a collaborative, student-driven approach that empowers students with disabilities to take an active role in shaping their educational and life goals. By focusing on each student’s strengths, preferences, and aspirations, PCP supports meaningful inclusion, promotes self-determination, and facilitates successful transitions to adulthood.
The process brings together educators, families, and support teams to co-create personalized action plans that reflect what truly matters to the student. At Empowered, two of our Core Team members are trained PCP facilitators, offering expert support to help your team implement this powerful approach with confidence and clarity.
